Case & Disease Management
We want to help you stay as healthy as possible. We are here to educate and support you in being an active partner in your behavioral health care. We can answer questions about your condition. We can also help you get the most out of your behavioral health, medical and pharmacy benefits. One way we do this is through our case and disease management programs.
Please note: Only members with case and disease management benefits through CBA can take advantage of these resources. Please see your Schedule of Benefits or talk to your human resources department to see if you are eligible for CBA's case and disease management services.
